Complete intakes, maintain electronic database and UPK registration if necessary.
Provide Behavioral Screening within the first 45 days.
Enter, track and follow up on all medical screenings including dental, hearing, and vision
Enter and track all physicals inclusive of immunizations and lead screenings in accordance to Article 47
Follow the Asthma Action Plan as needed for children
Consult with the Health and Safety director as needed.
Monitor family and child’s progress, maintain tracking forms and present information in multi-disciplinary Team meetings.
Conduct needs, assessments and assist families in accessing necessary supports and services using KHCConnect or other Inter-Agency Partnerships.
Support families in goal setting and achieving family goals via Florida Assessment
Support referrals to the Committee for Preschool Special Education (CPSE) and ensure CPSE Services are provided.
Provide referrals for therapeutic services for children and families with mental health concerns
Lead or co-lead parent workshops
Participate in classroom activities to support children and Teachers am/pm drop off and pick up, classroom observations for concerns and classroom coverage for Teachers including rest time.
Participate in staff meetings and staff training activities
Collaborate with community partners on shared projects
Any other duties as assigned by supervisor or Director
At minimum a credential or certification in social work, human services, family services, counseling or a related field.
