Social Worker Job Description

The Social Work Supervisor is part of the Early Childhood Leadership Team at the agency that understands and holds a Trauma Informed Lens of systems. Their role at Program Support LLC Family Child Care Network is to support the social, emotional and mental health of the affiliate providers, children and families. As part of the team they are responsible for supporting the purpose of the network, creating a welcoming holding environment for affiliated providers children and their families and increasing Program Support’s staff's knowledge and understanding of Infant/Early Childhood Mental Health. Through their leadership they are responsible for the provision of services, oversight of administrative processes, and direct supervision of the Family Workers team.

The Social Worker will provide early intervention services to infants and toddlers presenting with developmental delay or who have risk of delay due to environmental, medical and/or biological factors.

RESPONSIBILITIES Include But Are Not Limited To

Provide individual and group reflective supervision to the Family Workers in support of Affiliate Providers, children and their families.

Expand Program Support’s staff, affiliate providers and families understanding of trauma informed practices.

Meet weekly with Family Workers to support them in their work.

Meet one on one with Family Workers

Meet with Affiliated providers, children and families as needed.

Create trainings for affiliate providers, program staff and families on best practices for support social, emotional and mental health.

Make referral recommendations for services

Monitor health records of children and affiliate provider staff

Provide direct child developmental and family support services and case management to Affiliated Providers in home and community settings.

Function fully as part of interdisciplinary team.

Coordinate and participate in child developmental assessments and monitor conduct of additional child/family assessments and service delivery.

Facilitate and participate in development of IFSP (Individualized Family Service Plan), and in the execution, review and evaluation of the plan.

Collaborate with client families in identifying their concerns, priorities and resources.

Coordinate full range of home-based services with early intervention team.

Provide education, advocacy, and support on child and parenting issues and information of community resources to client families.

Refer for discrete/specialized child therapies and refer to other case management and counseling services when appropriate.

Coordinate intervention service with medical, state, and health providers with family consent.

                       

Requirements

Education: Master’s Degree in Social Work (MSW) from an accredited school required. Licensed Clinical Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) licensed by the Board of Registry of the New York State.

five+ years of supervisory experience is required.

Fluency in Spanish    

Individual, group, and family intervention skills required.

Team-building, communication, organizational and computer skills required.

Ability to work evenings and weekends.
