I have read and understand the requirements for participation of the NYC Early Childhood Apprenticeship program.
I understand that I am agreeing to participate in the NYC Early Childhood Apprenticeship program.
I understand that I agree to work at least 28 hours per week in an early childhood program or child care center in New York City and enroll in CDA program.
I provide consent for the Career Center to maintain contact with me over the next two years to follow my progress and employment status.
I grant permission to the Career Center staff to access and review my Aspire profile information for the purposes of providing career guidance, resume feedback, and professional development assistance. I understand that this access will be used solely for educational and career support services.
If selected as an Apprentice Candidate, I understand my name, contact information and resume will be shared with CUNY SPS and our employment partners.

Photography and Video Release Acknowledgement

I acknowledge the below photography and video release statement.

I hereby authorize the New York Early Childhood Professional Development Institute (The Institute) under the auspices of The Research Foundation of the City University of New York (CUNY) to take and use photographs of me in connection with the above-identified event. I authorize The Institute/CUNY, its assigns, and transferees to copyright, use, and publish the photographs in print and/or electronically.

I agree that The Institute/CUNY may use such photos of me with or without my name for any lawful purpose, including but not limited to publicity, illustration, advertising, and web content.

I understand that this event may be recorded and shared online. Any questions asked or comments offered over audio or entered in the chat box may be included in the recording.
